Dear Robert

 

Please let everyone know that the answer I received is that everyone who went in 1975 and has been active in the Church is qualified and must submit their forms through their current Regional Presidents.  So, for example, in your case you would submit your form to the European Regional President (continental leader)  

 

About having received a prize before, that is something the Regional President has to ask case by case to for FFWPUI HQ.
